To improve my services, I ask my prior clients to give me feedback using an anonymous survey.  The following are comments from surveys I've received, with scans of the actual survey so you can read the comment in context.  

Note that not all clients return surveys or permit me to publish their remarks.  However, those who do get their survey posted.  I do not filter out negative reviews.  So long as the client expressly permits me to use the survey, I do so.
